The new documentation system will allow the acute stroke team to update patient progress daily without generating a new form each day. This will create a continuous, accessible patient record that follows stroke survivors from admission to discharge (either home or to a rehab facility). The improved documentation process is expected to streamline information sharing, reduce redundancy, and enhance care coordination. This project would benefit the organization for the next Regional Stroke Accreditation.
Implementation of a Cerner-based documentation template for acute stroke team updates.
Improved continuity of care through a single, updatable patient progress record.
Enhanced collaboration and communication between acute stroke, nursing, and allied health teams.
Reduction in documentation redundancies and better tracking of patient progress.
A pilot study assessing the effectiveness of the new documentation system
Student Involvement:
System Analysis & Design: Assessing current documentation challenges and workflow inefficiencies.
Cerner Integration: Collaborating with IT and clinical informatics to design and implement the new documentation template. (can review with stroke team how the current form is used)
Training & Implementation Support: Assisting with user training and support for nursing/allied health staff. (may be a separate project to develop training info)
Data Analysis & Reporting: Evaluating the effectiveness of the new system through user feedback and performance metrics. (may be outside the project timeline but could develop the evaluation tool?)
