No skills listedPurpose: to empower patients by optimizing and creating patient education that aligns with best practices and current practices at HHCC
Expected impact: At HHCC there are opportunities to update current patient education while in some areas there is a need for new patient education since HHCC recently has implemented new quality initiatives through new innovation equipment and procedure, and processes. The specific focus of this project will be in 3 clinical areas:
a. Ambulatory Care Clinics- develop discharge patient education materials specific to each clinics
b. Operating room and Surgical Units - develop discharge instructions for specific procedures
c. Inpatient /outpatient units – update current admission handbook
The aim of this project is to develop updated patient education handouts and materials in addition to verbal counseling of patients to help understand the disease conditions, procedure, proper advice or on next steps, and when to call for help.
The aim is also to provide staff on how to access these education materials on a day to day basis
1. Work with the educator and manager of each area to complete a gap analysis of what the current education materials are and compare these with best practices and peer hospitals.
2. Will create a draft patient education using best practices and ensuring its relevance to HHCC. During this stage they would work with educators, scholars, Managers, physicians, and Patient Family Advisor for stakeholder input
3. They will be follow the approval process of patient education materials and work with communication team to ensure reader and audience appropriateness.
4. Once materials approved will provide rounding and education to clinical staffs on the education materials on purpose, where it is located, and go through the education materials with staff and family/patients.
5. At the end of the project the student will work on a qualitative survey to collect feedback on the education materials that can be used to measure the project success
